Community Mental Health Hub Service

Our Community Mental Health Hub service aims to deliver excellent integration, which is recovery focused and evidence-based health and social care to the residents of Weston-super-Mare, Worle and its surrounding villages.

The role of the team includes the following:

  • Assessing, treating, and reviewing symptoms.
  • Taking optimistic views about recovery focused interventions.
  • Providing a wide range of psychosocial interventions and support.
  • Providing support and intervention for families and carers.
  • Working closely with local practices to provide joined up Mental and Physical Health Care.
  • Working in partnership with a range of Statutory and Non-Statutory services to support our locality and population.
  • Assessing social care needs and providing social care services through personalised care and budgets.

Working in close partnership with One Weston Locality, our Community Mental Health Hub is now in motion. Our Hub brings together a range of mental health and multi-disciplinary specialists such as Therapist, Pier Support Workers, GP’s, Social Workers, Recovery Navigators, Psychologists and Specialist Navigators and is based around our surgeries to give access to special Mental Health support.
